Petro-Canada completes Terra Nova incident investigation; equipment failure cited

HOUSTON, Nov. 29 -- Equipment failure was the primary cause of a discharge of oily water from the Terra Nova FPSO Nov. 21, along with "a combination of factors," said operator Petro-Canada (OGJ Online, Nov. 24, 2004).

Petro-Canada submitted a detailed preliminary report with that finding to the Canada-Newfoundland Offshore Petroleum Board and said the company is "cooperating fully with the C-NOPB as it conducts its own investigation."

Petro-Canada said it is taking "immediate corrective measures, including equipment upgrades and process improvements," and is looking at other, longer-term options to improve its systems and procedures to prevent future incidences.
