Unsafe work practices caused a 2006 fire and explosion in a Mississippi oil field that killed four employees of a contractor and seriously injured a fifth employee, the US Chemical Safety Board concluded in a recently released investigation.CSB recommended that the Occupational Safety and Health Administration increase inspections of the region’s oil and gas production facilities.
